From time to time we need to update our membership information as, well the church is usually the last place you think to notify of a change of address, new phone number, name change, or even to transfer your membership.  Below is a form that you can download and fill out for our clerk. This information is also used to create and update a membership directory. You can choose what information is shared in the directory.

1. Download a copy of the editable PDF below.

2. Fill out your information and SAVE or PRINT the PDF.

3. Email a copy to Kimberly Hill (moyah2016@gmail.com) or hand it to her in person (if printed).
